Visitor Agreement

Everyone entering the climbing areas needs a waiver on file—including climbers, spectators, and belayers.

INSTRUCTIONS FOR PARENTS FILLING OUT A WAIVER FOR A MINOR: 
All waivers for minors include the adult AND the child. Adult Name AND Adult Birthday must be entered first.
Child's information will be entered later. Follow these step-by-step instructions:

1. Start by selecting "1" Adult (you) and the number of minors you’ll be signing for. Remember, you MUST be the parent or legal guardian of any minors you sign for. Then tap the "Continue" button.

2. Under “Agreement Details”, you’ll select YOUR NAME from the first dropdown menu and then for each minor select the teal “+ Add New Customer” button in the row(s) below to add the minor(s) information (name and birth date).

3. Continue scrolling down to fill out more information in the sections titled "Customer Information", "Address", and "Additional Information".

4. Finally, tap the “Continue” button to read through and sign the Visitor Agreement. Initial and sign all areas requested in the form.

IMPORTANT!
1) Waivers for minors must be signed by a PARENT (or COURT APPOINTED legal guardian). If the parent isn't present, they can fill out the waiver online and email us a photo of their ID to climb@wilkesbarreclimbinggym.com or send a copy/photo of their ID with their child.
2) Waivers signed by a friend, relative or group leader WILL NOT BE ACCEPTED and the child in question will have to sit out of the activities.
